Description of Services: Created to meet the needs of frail older adults who have been hospitalized and are at risk of readmission and further frailty * develops an individualized care plan for individuals, based on common health factors such as medication management, mobility and balance challenges, chronic pain, urinary incontinence, hearing loss, visual impairment, weight loss or malnutrition, depression, dementia, and caregiver burden and burnout

  • short term outpatient assessment and rehabilitation for seniors and adults with multiple health concerns
  • assists individuals to achieve optimal health, maximize independence and remain in their home
  • individuals typically visit the day hospital for 1-3 hours once or twice a week for 4-8 weeks
