Eligibility / Target Population

Residents of Sudbury Manitoulin catchment area 16 years and older with a Life expectancy of less than 3 months * Palliative Performance Scale (PPS) of 50% or less * do Not Resuscitate Certificate (DNRC) * valid Ontario Health Card * client of Ontario Health atHome * priority will be given to clients who live alone and cannot manage at home

Dedicated to providing quality hospice palliative care * helps individuals and their families realize their full potential to live even when they are dying, by attending to their physical, psychosocial, spiritual, and practical needs, in the location of their choice

Residential Care Program

provides 24-hour nursing and personal support care to end-of-life residents, including physicians, pharmacists, visiting volunteers, supportive care and bereavement coordinator
focus is on pain management, symptom control and compassionate support
