Support services for people with long term health care needs Respite Program - Overnight respite support to caregivers so they may be away from home Caregiver Support Program - Helps to maintain the well-being of a caregiver caring for a loved one * 24-hour Seniors Attendant Care in-home helps with medications, meal preparation, personal care, ROM exercise, and foot care PATH Program - Assists older adults in transition from hospital to home where a personal support worker will assist the older adult to settle in at home and to obtain food and medications |
