  • purchase of foot care/chiropody services for eligible cases * services requires a medical diagnosis of diabetes
  • financial support to secure transportation to dialysis appointments.
  • purchase of resources to mitigate the symptoms of diabetes
  • advocacy and system navigation to appropriate community education resources
