Description of Services: | This program is designed for pregnant women, mothers, or fathers with children under the age of six. All Pregnancy/Parenting Outreach Program clients are assessed and involved in the treatment planning process. Staff provide counselling and referrals in the client's home. | ||

Application: | Self-referrals accepted. Call, email or complete the online referral form. Following a telephone assessment and information session, individuals will be assigned to an outreach worker. | ||
