Organization: Noojmowin Teg Health Centre - Little Current
Description of Services: Works with indigenous population within Manitoulin catchment area, experiencing possible cognitive and/or functional decline, dementia and other mental health related difficulties impacting on quality of life. Provides psycho-social assessments, incl. financial, social, environment, mood, memory, cognitive, physical and mental health, as well as activities of daily living (ADL). Services also include case management, advocacy, referrals to other agencies, support to caregivers/significant others/family. Works in collaboration with aging at home supports and client’s circle of care. Provides recommendations for intervention to enhance quality of life, counselling support and ongoing monitoring. Educational support provided regarding advanced care planning.

Application: Self-referrals accepted as well as from physicians, family, community nurses & other community organizations.
