Description of Services: |
Provides outpatient rehabilitation services * programming includes interventional procedures, time-limited and goal-oriented rehabilitation services (up to 6 months) Ontario Telemedicine Network is utilized to provide some rehabilitation interventions, consultation and follow-up services to minimize the need for travel Pain duration must be lasting longer than three months and patient has not responded to other pain management therapies * chronic pain conditions include: The interdisciplinary team consists of the Clinical Manager, Program Coordinator, Clinical Lead, Administrative Support, Case Manager, Occupational Therapist, Pain Specialists - Physicians, Pharmacist, Physiotherapists, Recreation Therapist, a Psychological Associate, Registered Nurses, and a Registered Social Workers |

Eligibility - Population(s) Served: | Adults 18 years and older with a chronic pain condition | ||
Application: | Referral by family doctor or nurse practitioner * referrals for clients who have active claim or appeal for WSIB or motor vehicle accident are not accepted | ||
