Must be referred by Timmins and District Hospital, Anson General Hospital, Lady Minto Hospital, Bingham Memorial Hospital, Smooth Rock Falls Hospital, Sensenbrenner Hospital, Notre dame Hospital, Chapleau Health Services

Available to older adults being discharged from inpatient units or emergency departments
