Program: Geriatric Social Worker

Description of Services: Geriatric Social Worker works collaboratively with the other health care providers to enhance quality of life for First Nation elders * helps with the psychosocial needs of First Nation members working collaboratively with their families

Assists individuals who are experiencing a functional decline, difficulties with activities of daily living, cognitive difficulties, behavior changes, mental health issues complicated by age-related decline and those who require advocacy to connect to appropriate community resources * meets with First Nation elders in their home or at one of the community health centers and provides education to clients and caregivers, helps First Nation elders live independently by linking them to community services for ongoing support, provides assessment and memory testing, provides assistance in the development of behavior management strategies and offers supportive councelling for First Nations elders and their loved ones
