Description of Services: Paramedics can use the PERIL tool to determine risk, and make direct electronic referrals for community support services

  • free CPR and AED training sessions offered at various locations with the goal to increase bystander CPR and use of public access defibrillators in sudden cardiac arrests
  • monitor and address electronic referrals to ensure appropriate community partners have been notified of situations that pose an elevated risk to person or family and require multi-agency response
  • provide regular wellness clinics at emergency shelters for the homeless and help reconnect them to health services
  • community health assessments clinics to identified subsidized housing complexes with a focus on early identification and prevention for chronic disease * participants are monitored weekly, provides education, results are shared with family practitioners
