Description of Services: This program provides intensive community treatment, rehabilitation and support to individuals living with a severe and persistent mental illness. Focuses include improving mental health symptoms, increase functioning and achieving treatment goals. Services include working with the Forensic Outreach Program to serve those individuals bound by an order of the Ontario Review Board.

Application: All referral sources accepted including self-referrals. Complete the online form or call.
