Description of Services: Transitional Housing helps people regain their independence following a period of crisis by offering support services and accommodations * services include:

In-Home Personal Support: for the activities of daily living, including medical and non-medical personal care and meal preparation

Housekeeping: assistance with daily chores such as laundry, cleaning the bathroom and kitchen, vacuuming, dusting, mopping the floor and laundry

Respite: temporary relief for caregivers caring for people with minor medical conditions including personal care, light housekeeping, attendant care, and monitoring

Transportation: non-emergency transportation to local medical appointments and regular activities such as grocery shopping and banking

Companion Care: in-home visits for people who live alone, to provide companionship, pet care, errands, shopping, and meal preparation

Short term accommodations: for people who have overcome a crisis and are coordinationg services for their next level of care

About Us

Transitional Home Health and Transition House work together to deliver services within the comfort of your home or within our Sudbury Transition House locations. We provide short term accommodations to our guests who are convalescing, require respite services or are receiving treatments at HSN or the Cancer Treatment Center.
