Description of Services: Provides additional health services to community members that can include:
  • Assistance with filling out various forms (Ontario Works, Employment Insurance etc)
  • Information and counselling about smoking cessation
  • Information regarding Power of Attorney forms
  • Chronic disease self-management groups
  • Immunizations
  • Cancer Screening
  • Cholesterol Management workshops
  • Blood Pressure (BP) Clinics
  • Walking Group
Doctor-referred programs available include:
  • Senior's Health Day
  • Cardiovascular Risk Assessments
  • Spirometry testing and Asthma/COPD counselling
  • Ankle Brachial Index (ABI) Testing

Fees: None ; Most services covered by OHIP
Eligibility - Population(s) Served:
  • Patient must be rostered with Family Health Team to access most services; form available at front desk
