Description of Services: For individuals who have complex bio-psychosocial needs due to severe and persistent mental illness.

Intensive treatment, rehabilitation and support include; assertive outreach, 24-hour crisis service provided in conjunction with Sault Area Hospital Crisis Services, supportive counselling, medication monitoring, skills building and advocacy.

Application: Self referrals are accepted. Must meet ACT criteria. High prior hospitalization, emergency, and/or community involvement. Priority given to transfers from other ACT teams, long-term psychiatric units and schedule one facilities.
