Description of Services: The Behavioural Support Outreach Service (BSO) is integrated with the Geriatric Mental Health Service, providing outreach services to caregivers and individuals with responsive behaviours in long term care facilities, acute care, and in the community. Team members facilitate patient and caregiver support to improve care and quality of life of older individuals with behaviours associated with dementia or complex and challenging neurodegenerative cognitive disorders.

Eligibility - Population(s) Served: Older Adults Only
Application: Referrals are accepted by community organizations, long-term care facilities, families and caregivers. To make a referral, or for general inquiries, please call our toll-free number or fill in the referral form online. Referrals are processed through Behavioural Supports Ontario (BSO) - North East Central Intake.
