Description of Services: The Case Management program is designed to assist individuals with mental illness in maintaining and enhancing wellness. The program will assist clients who are transitioning from hospital to the community. This is an outreach service which helps clients to achieve their personal goals and links them with appropriate services and supports in the community.

Application: Individuals may self-refer by phone or in person via central intake. Referrals are also accepted from other health care providers or families, with the consent of the person being referred. Referrals go through central intake. Clients will be contacted for screening/assessment.
