Program: Moonlight Avenue

Description of Services: The Moonlight Avenue housing program will provide supportive living to eight individuals. It will serve a population group primarily from North Bay Regional Health Centre (NBRHC) and Health Sciences North (HSN) that is confronted with severe and persistent mental health issues; those experiencing long hospital stays, or those who make frequent use of hospital facilities. Through effective and comprehensive discharge planning, tenants wishing to return to their community of choice will now have that opportunity.

The Moonlight Avenue home will reflect a self-directed/supported model encompassing comprehensive case management, peer support and individualized wellness plans in preparation for community living. Tenants may actively participate in day-to-day planning and decision making and will undertake a wide range of daily living, recreational and social activities while living at the home.

Application: Referrals come from North Bay Regional Health Centre (NBRHC), Health Sciences North (HSN) and then community. Individuals submitting applications for tenancy are required to have case management services while residing in the home. Tenant agreement packages will be reviewed and signed by both parties prior to moving in. Tenant handbooks are provided, outlining house rules and expectations.
