Program: Court Outreach/Justice Program - Sudbury - Elm Street

Description of Services: The Justice Program supports individuals with mental health and/or substance use concerns who are involved with the criminal justice system. The program provides a wide range of services for adults, including court outreach and navigation, connecting people to mental health supports and essential community services. After eligibility screening, individuals are paired with a Justice Case Manager who assists with personal goals, care planning, and strategies to support recovery and reduce re-offending. Together, we work with legal counsel, families, and the courts to support individuals 18 and over with bail release planning, court processes, mental health diversion screening, and accessing forensic fitness screening and assessments, if necessary. Mental Health Diversion is a voluntary program that provides support to individuals who have been charged with minor offences, where mental health has been determined to be a factor in the offence. Individuals who are eligible will receive support with community referrals, advocacy, education, and goal planning to support their recovery. Successful participation may result in charges being withdrawn or the individual receiving an alternative legal resolution.

Application: Self-referrals accepted for Court Support services only. Diversion referrals come from the courts only. Call or complete the on-line form to initiate contact and arrange for an intake.
