Description of Services: Helps individuals make the transition from hospital to home safely, smoothly and comfortably * includes transportation home and the support of a Personal Support Worker from discharge through settling in once at home * helps to settled the individual in and stays with them for a prearranged amount of time or until family arrives * provides follow ups and helps with other services that may be required to ensure the safety of the individual * goal is to keep the family member safe and healthy at home
