Must be referred by Health Science North, St Joseph's Continuing Care Centre, Espanola Regional Hopspital and Health Centre and Manitoulin Health Centre

Eligibility / Target Population

Older adults 55 years and older, able to direct their own care, medically stable and can be left unattended * must be independently mobile (unless family/caregiver is available to provide physical assistance) and have been admitted (or discharged from) an inpatient unit or have presented at the Emergency Department
