Description of Services: Helps patients make the transition from hospital to home * works to coordinate, implement, assist wherever possible, to ensure a successful stay at home

Fees: Free
Eligibility - Population(s) Served: Older adults 55 years and older, able to direct their own care, medically stable and can be left unattended * must be independently mobile (unless family/caregiver is available to provide physical assistance) and have been admitted (or discharged from) an inpatient unit or have presented at the Emergency Department
Application: Must be referred by Health Science North, St Joseph's Continuing Care Centre, Espanola Regional Hopspital and Health Centre and Manitoulin Health Centre
