Description of Services: Social and recreational activities in a supervised group setting for older adults and adults with physical disabilities either living alone or with a caregiver * wellness oriented program including assessment and monitoring * benefits include companionship and peer support * individualized program planning * assistance with activities of daily living * therapeutic social/recreational activities * safe routines and environment * respite for the caregiver

Fees: $15 a day with meal * $7.50 half day with meal
Additional fee for transportation * subsidies may be available
